The first visit to our office:
We want to make you feel comfortable and relax . Please arrive 20 minutes earlier to your first appointment in order to fill some important forms such as the personal information, the medical history and the dental insurance information or if you prefer you can always request all these documents to be mailed to you so not need to spend extra time filling information upon your arrival. Please note that we need at least 7 days notice in advance for mailing forms.
When you make your first call to book in an appointment please have your Dental Insurance information with you along with Group number, Policy number, Insurance telephone number, Date of Birth and Social Security info are needed.
In you suffer from any medical condition, it is also important to bring with you all the information of your physician such as: name of physician, office telephone number and fax number.
According with the American Dental Association. Patients with the following medical treatments or conditions are required to take antibiotics at least ONE HOUR before any dental treatment including Teeth Cleaning.
- Total Joint replacement including Hip and Knee.
- History of Infective Endocartitis.
- Artificial heart valves.
- Congenital heart defect with prosthetic device.

If you think that a pre-medication is not needed we might need a authorization from your physician. You can bring it with you or have them faxed to (408) 262-9998.
In case of cancellation please give us a call at least 24 hours before your appointment. Thank you for your confidence in our office, we are looking forward meeting you and to assist you and your family with all your dental needs.

Tu primera visita;
Queremos que te sientas comodo y tranquilo. Favor llegar 20 minutos mas temprano de tu cita para que tengas el tiempo suficiente de llenar los formularios de historia personal, historia medica y seguro dental. Si prefieres puedes solicitar estos formularios para que sean enviados por correo a tu casa, asi no tienes que dedicar tiempo extra llenandolos en la oficina el dia de tu primera cita. Recuerda que nesecitamos al menos 7 dias de aviso para el envio de los formularios via correo postal.
Cuando llames para reservar tu cita con nosotros, favor tener a la mano la siguiente informacion: Seguro Dental ( si es que tienes ), numero de poliza del seguro, telefono del seguro, tu fecha de nacimento, y tu numero de seguro social ( si es que tienes uno )
Si presentas algun tipo de condicion medica o algun tipo de tratamiento medico que requiere tomar antibiticos antes de cualquier tratamiento dental inclusive limpieza dental, favor traer contigo lo siguiente: nombre del medico, telefono del medico y el numero de fax.
Segun la Associacion Americana Dental, las siguientes condiciones medicas necesitan la administracion de antibioticos al menos UNA HORA antes de cualquier tratamiento dental inclusive Limpieza dental.
- Remplazo total o parcial del hueso de la Cadera o Rodilla con protesis.
- Historia de Infective Endocartitis o Inflamacion de la pleura del corazon.
- Pacientes con Valvulas cardiacas artificiales.
- Pacientes con Defecto cardiaco al nacer y que tienen protesis instalada.

Si tu consideras que no necesitas la administracion de antibiticos, necesitamos la informacion de tu medico para llamarlo.
